Uchadza? musi sp??a? podmienky u?asti uvedene v 32 ods. 1 zakona ?. 343/2015 Z. z. o verejnom obstaravani a o zmene a doplneni niektorych zakonov v zneni neskor?ich predpisov. Ich splnenie preuka?e pod?a 32 ods. 2, resp. pod?a ods. 4 a/alebo ods. 5 zakona predlo?enim originalnych dokladov alebo ich uradne overenych kopii v rozsahu a platnosti v zmysle zakona o verejnom obstaravani alebo v zmysle 152 zakona o VO.

Verejny obstaravate? stanovil podmienku u?asti v sulade s 34 ods. 1 pism. a) zakona o verejnom obstaravani a primerane k predmetu zakazky, vzh?adom na to, ?e sa jedna o dodavku zdravotnickej techniky je pre verejneho obstaravate?a potrebne, aby uchadza? preukazal, ?e ma dostato?ne skusenosti s dodavkami tovaru rovnakeho alebo podobneho charakteru ako je predmet zakazky a je schopny zabezpe?i? in?talaciu, servis a udr?bu pristroja s prislu?nou odbornos?ou a v po?adovanej kvalite.

2. Pod?a 34 ods. 1 pism. m) ? 2 zakona o verejnom obstaravani.

Odovodnenie primeranosti:

Verejny obstaravate? stanovil podmienku u?asti v sulade s 34 ods. 1 pism. m) zakona o verejnom obstaravani a primerane k predmetu zakazky vzh?adom na to, ?e sa jedna o dodavku zdravotnickej techniky je potrebne, aby uchadza? preukazal, ?e disponuje po?adovanymi dokladmi, ktore su vy?adovane pre plnenie predmetu zmluvy v zmysle po?iadaviek na pou?ivanie zdravotnickej technologie pri splneni technickych noriem kladenych na predmetnu zdravotnicku technologiu.

Minimum level(s) of standards possibly required:

AD.1) Uchadza? splnenie podmienky u?asti preukazuje zoznamom dodavok tovaru rovnakeho alebo podobneho charakteru ako je predmet zakazky za predchadzajuce 3 roky od vyhlasenia verejneho obstaravania s uvedenim cien, lehot dodania a odberate?ov; dokladom je referencia, ak odberate?om bol verejny obstaravate? alebo obstaravate? pod?a tohto zakona.

Zoznam referencie musia obsahova? minimalne tieto udaje:

a) obchodne meno a sidlo predavajuceho;

b) obchodne meno a sidlo kupujuceho;

c) zmluvny termin dodania, skuto?ny termin dodania;

d) mno?stvo dodaneho tovaru (ak je to vzh?adom na sortiment tovaru mo?ne) alebo druh, pripadne sortiment dodaneho tovaru;

e) celkovu zmluvnu cenu, ktorou sa rozumie celkova zmluvna cena za dodanie tovaru. V pripade, ?e skuto?ne fakturovana celkova cena sa li?i od celkovej zmluvnej ceny a je preukazana pod?a ?alej uvedenych pravidiel, bude akceptovana tato faktura?na hodnota (?alej len cena).

Mena pre cenu: EUR.

Cenu je potrebne uvies? v ?trukture: cena bez DPH, DPH a cena s DPH pre jednozna?ne posudenie splnenia po?adovanej minimalnej urovne.

Cenu v inej mene ako je EUR je potrebne prepo?ita? priemernym ro?nym kurzom stanovenym Europskou centralnou bankou na dany rok, ku ktoremu sa kvalifikovana referencia via?e a uvies? ako bola vypo?itana;

f) meno a priezvisko, funkciu a telefonne ?islo alebo e-mail kontaktnej osoby odberate?a, u ktorej si mo?no overi? udaje uvedene v kvalifikovanej referencii

Su?et cien referencii za predchadzajuce tri roky od vyhlasenia verejneho obstaravania musi by? rovny alebo vy??i ako 800 000 EUR bez DPH.

AD.2) Uchadza? predlo?i ES vyhlasenie o zhode na ponukane zariadenie s dopl?ujucimi podkladmi k nemu, certifikaty vydane autorizovanymi osobami resp. notifikovanymi osobami Europskymi spolo?enstvami, ktore maju opravnenie na posudzovanie zhody vyrobkov, resp. ekvivalentne doklady. V pripade, ak uchadza? predklada ES vyhlasenie o zhode, v ktorom sa odvolava na ine dopl?ujuce dokumenty, ktore su podkladom na jeho vydanie, musia by? tieto dopl?ujuce dokumenty su?as?ou predlo?eneho ES Vyhlasenia o zhode.
