Zaji?t?ni vykonu ?innosti technickeho dozoru stavebnika (TDS) a koordinatora bezpe?nosti a ochrany zdravi p?i praci (BOZP) a poskytnuti souvisejicich slu?eb v ramci realizace nasledujicich stavebnich praci:

a) stavebniho objektu I, ktery je vymezen uzemnim rozhodnutim ?. 2901/2011-1 vydanym M?stskym u?adem Nachod, odborem vystavby a uzemniho planovani dne 23.5.2011 pod spis. zn.: 963/2011/VYST/LP/ s nabytim pravni moci 25.6.2011 a dokumentaci pro uzemni ?izeni zpracovanou spole?nosti Obermeyer Helika a.s., Beranovych 65, 199 21 Praha 9-Let?any (dale jen ?DUR) ve zn?ni pozd?j?ich p?ipadnych zm?n vzniklych p?i zpracovani dokumentace pro stavebni povoleni a pro provad?ni stavby,

b) realizace vym?nikove stanice s technologii para/voda v?etn? oh?evu TUV umist?ne v objektu J (SO 10) v mistnosti ?. J.01.009 p?vodni vym?nikove stanice s technologii voda/voda,

c) do?asna ve?ejna parkovi?t? vn? arealu nemocnice Nachod (parkovi?t? v ul. Nemocni?ni, parkovi?t? pod horni nemocnici), ktere jsou obsa?eny v PD

Toto pravo bude vyu?ito v p?ipad? pot?eby zadavatele a dle jeho rozpo?tovych mo?nosti. Vy?e uvedene stavebni prace jsou p?edm?tem vyhrazene zm?ny zavazku ze smlouvy na ve?ejnou zakazku s nazvem Oblastni nemocnice Nachod ? I. etapa modernizace a dostavby a budou p?ipadn? zadany do 3 let ode dne uzav?eni smlouvy na p?vodni ve?ejnou zakazku, proto i tyto nove slu?by budou p?ipadn? zadany do 3 let od uzav?eni smlouvy na p?vodni ve?ejnou zakazku a budou zahrnovat slu?by obdobneho charakteru jako je p?edm?t teto ve?ejne zakazky. Pokud zadavatel vyhrazenou zm?nu zavazku ze smlouvy uplatni, bude p?i sjednani ceny a podminek pro nove slu?by postupovat podle ustanoveni 100 odstavec 3 zakona ?i dle pravnich p?edpis? platnych v dob? uplatn?ni vyhrazene zm?ny zavazku ze smlouvy.

P?edpokladana hodnota zadavane ve?ejne zakazky ?ini 15 000 000,00 K? bez DPH a hodnota vyhrazene zm?ny zavazku podle 100 zakona (opce na dal?i slu?by) ve vy?i 4 500 000 CZK bez DPH.

Cena za vy?e specifikovanou vyhrazenou zm?nu zavazku podle 100 zakona nebude sou?asti nabidkove ceny u?astnika zadavaciho ?izeni.
