III.1.1) Suitability to pursue the professional activity, including requirements relating to enrolment on professional or trade registers
List and brief description of conditions:

A) Lettre de candidature (DC1 dans sa derniere version ou forme libre) permettant d'identifier le candidat individuel ou, en cas de co-traitance, le mandataire du groupement et chacun de ses co-traitants, dans les conditions precisees au sein du reglement de la consultation;

b) Documents relatifs aux pouvoirs de la personne habilitee a engager le candidat: Kbis de moins de 3 mois, et le cas echeant, delegation de pouvoir precisant les montants delegues;

c) Declaration sur l'honneur pour justifier que le candidat n'entre dans aucun des cas d'interdiction de soumissionner prevus aux articles 45 et 48 de l'ordonnance n 2015-899 du 23.7.2015 relative aux marches publics. La remise du formulaire DC1 vaut production de cette declaration sur l'honneur;

d) Declaration sur l'honneur pour justifier que le candidat est en regle au regard des articles L. 5212-1 a L. 5212-11 du Code du travail concernant l'emploi des travailleurs handicapes.

A) Declaration concernant le chiffre d'affaires global et le chiffre d'affaires concernant les travaux objet du contrat, realises au cours des 3 derniers exercices disponibles. La remise du formulaire DC2 vaut production de cette declaration;

b) Preuve d'une assurance pour les risques professionnels.

A) Selon les cas:

? la preuve de l'inscription a l'ORIAS pour les intermediaires d'assurance (agents et courtiers),

? le mandat de presentation, encaissement et gestion delivre par les societes d'assurance porteuses de risques (pour les courtiers dassurance),

? les agrements reglementaires pour les societes d'assurance, institutions ou mutuelles.

b) Declaration indiquant les effectifs moyens annuels du candidat et limportance du personnel dencadrement des 3 dernieres annees;

c) Liste des prestations de meme nature effectuees au cours des 3 dernieres annees, indiquant le montant, la date et le destinataire. Toutefois, l'absence de references relatives a l'execution de marches publics de meme nature ne peut justifier, a elle seule, lelimination du candidat.
