* Door in te schrijven op deze opdracht verklaart de inschrijver zich niet in een toestand van uitsluiting te bevinden, zoals bedoeld in artikels 67 tot en met 69 van de wet van 17.6.2016 betreffende overheidsopdrachten.

* In toepassing van artikel 62 van het koninklijk besluit van 18.4.2017 dient de Belgische inschrijver geen RSZ-attest bij zijn offerte te voegen. De aanbestedende overheid zal deze informatie zelf opvragen.

* Een lijst van de voornaamste diensten die gedurende de laatste 3 jaar werden verricht, met vermelding van het bedrag, de datum en de publiek- of privaatrechtelijke instanties waarvoor zij bestemd waren:

a) indien het diensten aan overheden betreft, worden de diensten aangetoond door certificaten die door de bevoegde overheid zijn opgesteld of goedgekeurd;

b) indien het gaat on diensten aan privaatrechtelijke personen worden de diensten aangetoond door certificaten opgesteld door deze personen of, bij ontstentenis, door een verklaring van de dienstverlener.

* Een verklaring betreffende de gemiddelde jaarlijkse personeelsbezetting van de dienstverlener en de omvang van het kaderpersoneel gedurende de laatste 3 jaar.

* De studie- en beroepskwalificaties van de dienstverlener en/of van het ondernemingskader en, in het bijzonder, van de verantwoordelijke(n) voor de uitvoering van de diensten.

* De inschrijver biedt een ploeg(en) aan met minstens 1 geschoolde begeleider (tuinbouw-/groenbeheersector) waarmee in het Nederlands gecommuniceerd kan worden.
