This notice is hereby amended to add brand name justification and sole source distributor letter.
The Billings Area Indian Health Service (IHS), Blackfeet Service Unit attends to award a non-competitive, sole source purchase order for a brand name Optos Wide-field Camera, model Daytona P200T to Melling Medical in pursuant to FAR 52.211-6, Brand Name or Equal.
The items specified in the attached technical specifications are for pricing estimate only. The associated North American Industry Classification System code is 339112 and the small business size standard is 200 employees.
PROJECT TITLE: Optos Daytona Retinal Wide-Field Camera and Dicom Modality Worklist and Live Store to Third Party PACS.
DELIVERY DATE: The Equipment shall be delivered to Blackfeet Community Hospital, 760 Hospital Circle, Browning, Montana within 90 days of award.
DELIVERABLES AND DELIVERY SCHEDULE: Within 90 days of award during the Weekdays Monday thru Friday between: 8AM – 5PM.
SCOPE: This equipment is used to evaluate the retina and is particularly useful in patients who do not wish to have pupils dilated. Retinal disease can easily be detected and documented using this wide-field camera as an essential part of the ocular health assessment.
