Description
The Division of Purchasing (DOP), on behalf of the Department of Health and Welfare Division of Medicaid, is
seeking proposals from qualified vendors to provide residential habilitation program coordination on a statewide
basis for Certified Family Home (CFH) providers who are providing residential habilitations services to adult participants
on the Developmental Disability (DD) Home and Community Based Services (HCBS) Waiver.

Group 1: Business Information
(ME) Business Profile- Provide a profile of your business including Offeror’s business history,

description of current service area, and customer base.
(ME) Experience - Describe in detail your knowledge and experience in providing services
similar to those required in this RFP. Offerors must have three (3) years of experience
providing services similar to those required in this RFP, including one (1) year in the human
services field. Offerors must provide detailed descriptions, including: 1) What type of
services did the Offeror provide that were similar to those required in this RFP; 2) When did

the Offeror provide the services similar to those required in this RFP; 3) Scope of project; 4)
Size of project (number served); and 5) Outcomes for each of the following:
1.2.1 Experience working with individuals with Developmental or Intellectual Disabilities;
1.2.2Experience writing habilitation programs;
1.2.3Experience providing skill training; and
1.2.4 Experience working with any State Medicaid program.
1.3
(M) Organizational Chart - Provide a copy of your organizational chart, including detail of any
relationships with parent and subsidiary organizations.
(ME) Demonstrated Success - Provide at least two (2) narrative examples that illustrate
previous success with 1.2.2. and 1.2.3. above. Specifically, how has your business entity

developed successful habilitation programs and successfully provided skill training to
providers. Also, provide at least two (2) additional narrative “Best Practice” examples of how
your business entity has provided services to people with Developmental and Intellectual
Disabilities as in 1.2.1 above.
(E) Customer Retention - Over the last three (3) years, approximately what percentage of
customers have chosen to remain with your company when given an option (e.g. exercising

optional renewals, extending agreements, selecting your company again upon re-solicitation,
etc.)
Faith-based Organization: If you are a faith-based organization describe how you will:
A.Segregate contract funds in a separate account;
B.Serve all participants without regard to religion, religious belief, refusal to hold a religious belief, or
refusal to actively participate in a religious practice;
C.Ensure that State-referred clients’ participation in religious activities, including worship, scripture

study, prayer or proselytization, is only on a voluntary basis;
D.Notify participants of the religious nature of the organization, their right to be served without religious
discrimination, their right not to take part in inherently religious activities, their right to request an
alternative provider and the process for doing so;
E.Ensure that contract funds are not expended on inherently religious activities; and
F.Comply with applicable terms of 42 CFR Parts 54 and 54a, and 45 CFR 260 and 1050.
Group 2: Organization & Staffing
(ME) Project Lead - Identify the person who will be the dedicated Project Lead if Offeror is

awarded a contract. Provide a description of the proposed Project Lead’s experience and
qualifications (or upload a resume in response to Question 2.2).

(ME) Key Personnel and Qualifications - Provide a list of key management, customer service
and other roles to be used in the fulfillment of this Contract (in addition to the Project Lead).

Provide role descriptions, including requisite qualifications and experience of the
person(s)/role(s) identified, as well as an explanation of how the person in that role will
contribute to the project. Your response should demonstrate the extent to which you have
the expertise to accomplish the Scope of Work.
(M) Subcontractors - If you intend to utilize subcontractors, describe the extent to which they
will be used to comply with Contract requirements. Include each position providing service,
and provide a detailed description of how the subcontractors are anticipated to be involved
under the Contract. Include a description of how the Offeror will ensure that all

subcontractors and their employees will meet all Scope of Work requirements. NOTE: The
information provided for subcontractors, if any, will be evaluated as part of Section 2.3, Other
Key Personnel and Qualifications. If you do not intend to utilize subcontractor(s), provide a
