Description:

The Department of Human Services (DHS), Office of Mental Health and Substance Abuse Services (OMHSAS), is seeking contractors to supply Outpatient and Emergency Services to Norristown State Hospital, as described in the attached Statement of Work.
Quantities/dollar amounts listed are estimates and may increase or decrease based on the needs of the facility.
Award will be made to multiple vendors found to be both responsive and responsible. The number of vendors awarded is at the discretion of the Commonwealth based on the needs of NSH.
Bids are due 5/23/2023 by 10:00 AM and must be submitted electronically. Late bids will be disqualified. Contractors must enter firm prices for all contracted years and provide appropriate responses to all “Mandatory Attributes” contained in the solicitation. Failure to submit bid as described may result in yours being rejected as a non-responsive bidder.
Cost for all bids should reflect a percentage discount based on the Contractor’s customary and usual charges. The Contractor’s percentage discount will be based on the Contractor's submitted Discount Off Usual and Customary Charges Certification Form. The Contractor is required to submit the Discount Off Usual and Customary Charges form, which also identifies the percentage you will be requesting if you choose to add a percentage. Quantity on the bid is an estimated five year dollar amount. Supplier bids should be bid $1.00 each. Please enter your plus/minus % rate on the Discount Off Usual and Customary Charges form. If you choose not to add a percentage, please enter 0% on the Discount Form. Invoice prices will be based off the Discount Form filled out and the vendor's price list.
The service will be provided by a healthcare organization that meets the standards set forth by the Centers for Medicare and Medicaid services.
The term of the contract will begin on the Effective Date and will expire five years from the Effective Date.

Contractors’ electronic submission must include the following as attachments. Failure to submit these items may result in the rejection of the Contractor’s submission:
1. Discount Off Usual and Customary Charges Form
2. Lobbying Certification Form
3. Workers Protection Form
4. Vendor's Point of Contact Information supplied on Statement of Work. The contractor shall designate a person who shall be familiar with the contract and authorized to act on the contractor’s behalf in resolving any issues relating to the contract and who shall be available to the facility during regular business hours and emergency requests.
