Item Information
Item # 1: ( 948 - 93 ) Removal of medical waste from Murray Developmental Center campus Oct. 1, 2020 thru June 30, 2021. Contractor will provide 28-gallon lidded bins clearly marked with a red biohazard symbol. Contractor will supply Center with 54 bins to begin (6 per location). Center may move bins to locations that may have increased need. Center may request from Vendor additional bins if need arises. Contractor will pick up any bins that contain infectious waste (no maximum weight per bin) and replace with empty bins. Vendor will leave bags for the inside of bins for Center staff to place in bins before putting infectious waste in bins. Contractor will pick up at 9 locations (Apple, Berry, Cherry, Daisy, Elm, Fir, Grape, Main Dietary, and Administration Building) on campus twice per week (Monday and Thursday between 8:00 am and 4:00 pm). Contractor will stop at each location on each pick-up day to check each bin. Vendor will sign in at Administration Building upon arrival. After pick-ups, Vendor will stop at Administration Building to have manifest signed and to sign out. Vendor must supply to Center Illinois EPA form IL520.2072 Illinois Potentially Infectious Waste Manifest with number of bins and pounds picked up. This will be used to verify pounds billed on invoice. Estimated pickup per month is 1000 pounds. Vendor must have an Illinois Environmental Protection Agency certification/permit to transport hazardous waste. Vendor shall comply with the Illinois Prevailing Wage Act and certify by signing the attached Form 30 ILCS 500/25-60 Prevailing Wage.
