COUNTY OF HUDSON DEPARTMENT OF HEALTH AND HUMAN SERVICES OFFICE OF DISABILITY SERVICES 2018 HUDSON COUNTY PEER GROUPING REQUEST FOR PROPOSALS Notice is hereby given that sealed proposals will be received by the County of Hudson, State of New Jersey Attention: Purchasing Agent, 3rd Floor 567 Pavonia Avenue Jersey City, NJ 07306 on Thursday, October 26, 2017 no later than 11:00 AM prevailing time, and at that time and place the following will be publicly opened and read for: The 2018 Hudson County Peer Grouping Request for Proposals will fund services that support Hudson County residents with disabilities, and other special populations. Specifically, these services will: promote self- sufficiency and stability among at-risk families; assist children with developmental disabilities in accessing needed care; and enable people with disabilities to remain within the community as independently as possible. Proposals are being sought for services that support Hudson County residents with disabilities, and other special populations, to maintain self- sufficiency within the community, access needed services and avoid institutional care. These services include: Independent Living Case Management; Transition Case Management; Assistive Technology; Hospice; Respite; Community Support; Child Care for Special Populations; Community Emergency Psychiatric Services; Early Intervention Program; Therapeutic Recreational Services; and Social and Educational Support Services. The grant period for this RFP will be January 1, 2018 through December 31,2018. A pre-application meeting will be held on Monday, October 2, 2017 at 10:00 a.m. at the Hudson County Department of Health and Human Services Building, 830 Bergen Avenue, Third Floor Conference Room, Jersey City, NJ 07306. Questions are to be submitted in writing via facsimile by Wednesday, October 4, 2017 by 4:00 p.m. to Maria Mercurio at (201) 369-4361. No further questions will be accepted after this time. Answers and addenda will be made available publically on Friday, October 13, 2017 at 9:00 a.m, Proposals shall be submitted on the forms provided and shall be in accordance with the RFP. Requests for Proposals may be viewed and obtained through the Hudson County Purchasing Department website, http://www.hudsonprocure.org. Applicants must register on the Purchasing Portal in order to download the request for proposals (RFP). Sealed proposals must be delivered to the County of Hudson, State ofNew Jersey, Attention: Purchasing Agent, 3rd Floor 567 Pavonia Avenue Jersey City, NJ on Thursday, October 26, 2017 no later than 11 :00 a.m, prevailing time. Six (6) proposals and one (1) CD/DVD or USB Drive which contains a secure copy of the complete proposal in PDF, WordPerfect, or Microsoft Word format must be hand-delivered at the designated time for receipt of proposals. All proposals must be contained in a~ envelope or box. Required language to be written in ink on the exterior of all envelopes or boxes is as described in the General Conditions and Instructions to Applicants. No late, emailed or faxed bids will be accepted. Applicants are required to comply with the requirements of PL 1975, c. 127 (NJAC 17:27) Affirmative Action Plan, Hudson County Living Wage Ordinance Nos. 363-6-2014, 364-6-2014 and PL 1977, c. 33, Disclosure Statement, and all other requirements set forth in the General Conditions and Instrnctions to Applicants.
