Applicants eligible to apply for this cooperative agreement are limited to existing grantees under FOA PS08-802 HIV/AIDS Surveillance, Part I Core Surveillance. Eligible applicants are state health departments, which includes the six independently funded local health departments (Chicago, Houston, Los Angeles, New York City, the City of Philadelphia and San Francisco), the District of Columbia, and the territorial health departments (Commonwealth of Puerto Rico, the U.S Virgin Islands). Competition is limited to existing grantees under PS08-802 to ensure that the HIV/AIDS surveillance system covers the entire United States and its territories. The eligible health departments have jurisdiction over promulgating laws and legislations over specific states and metropolitan areas. Only entities that are legislatively charged to administer and enforce state laws, rules, or regulations pertaining to collecting, protecting, and maintaining HIV surveillance data are eligible to apply under this announcement. All of these health departments were eligible to conduct core surveillance activities under PS08-802 (FY2008-FY2012).

Description
The purpose of the supplement to the HIV surveillance program cooperative agreement is to enable state and local health departments funded under FOA PS08-802 HIV/AIDS Surveillance, Part I Core Surveillance to apply for one or both of the following enhanced activities: Activity I: Laboratory Reporting - This activity will enable funded areas to improve the reporting of HIV-related laboratory data with special emphasis on Western Blot (WB), CD4+ T-lymphocyte (CD4) test and viral load (VL) detection test results from private and public laboratories. This activity may also include enhancing the reporting of other confirmatory, screening, and viral resistance tests. This activity will enable areas to measure, integrate, and evaluate testing initiatives by assessing new diagnoses, characteristics of persons newly testing positive, and linkage to and retention in care. Activity II: Geocoding Data - This activity will enable funded areas to capture, store, analyze, and display spatial data. Geocoded data can be spatially linked to other data sources to enhance understanding of social determinants of health (SDH) affecting communities impacted by HIV. A major use of geocoded HIV surveillance data is in monitoring and analyzing community viral load information and stage of disease at diagnosis. This information is needed for analysis of access/utilization of HIV care services and unmet need. Additionally, this data will assist jurisdictions in targeting and mapping testing sites in relation to need.
