GENERAL INFORMATION
PURPOSE OF REQUEST FOR PROPOSAL (RFP)
The South Dakota Department of Health (DOH) is seeking to pursue a partnership to create a medical respite program serving Pennington
County, South Dakota. This program must provide services to individuals experiencing homelessness. The program must be focused on
decreasing inappropriate healthcare system utilization, providing infrastructure to support the community during system strains, improving
community population health and well-being, providing cost savings, and ensuring job creation.
ISSUING OFFICE AND RFP REFERENCE NUMBER
The South Dakota Department of Health is the issuing office for this document and all subsequent addenda relating to it, on behalf of the
State of South Dakota, Department of Health. The reference number for the transaction is RFP # 24-0901005-023. This number must be
referred to on all proposals, correspondence, and documentation relating to the RFP.

SUBMITTING YOUR PROPOSAL
All proposals must be completed and received by the South Dakota Department of Health by May 1, 2024 by 4pm CT.
Proposals received after the deadline will be late and ineligible for consideration.
Proposals should be emailed to Kaitlin.Thomas@state.sd.us.
All proposals must be signed by an officer of the responder legally authorized to bind the responder to the proposal on the form intended
by the respondent. If the bidder chooses to submit an electronic copy, an electronic signature may be provided. Proposals that are not
properly signed may be rejected. The sealed envelope should be marked with the appropriate RFP Number and Title. Proposals should be
addressed and labeled as follows:

The State will to respond to offeror’s inquiries (if required) via e-mail. In addition, all inquiries and the State’s response will be posted on the

or other term or condition of this RFP. Offerors will be notified in the same manner as indicated above regarding any modifications to this
RFP.
