Performance Work Statement
Dental Surgical Guide Services for Guided Implant Surgery
Background:
This Performance Work Statement outlines the responsibilities and services to be provided by the Contractor in relation to the supply of CT Surgical Guides. These guides will include Collaborative Surgical Planning, the Surgical Guide itself, an optional temporary restoration (when required), and a detailed surgical report tailored for each patient. This service is crucial in facilitating optimal guidance during implant surgeries conducted at the Houston VA Dental Service and the Conroe Outpatient Dental Clinic.
The Contractor is responsible for the provision of all necessary materials and shipping packages for each case upon its submission with the requisite specifications. Upon receipt of a patient case or models, they are to be expediently transported to the Contractor s laboratory for processing. The Provider or Specialist initiating the request will oversee the coordination and manage the timeline for return. They are also responsible for handling any queries, needs, or addressing potential delays.
The proposed contract is for one year 12-month period with (4) 12-month option periods, if exercised.
Routine Submission:
Surgical Guides will be requested as per individual patient requirements, which may vary. The Contractor is obligated to deliver the results within 48-72 hours of the submission by the dental Provider/Specialist, or as specified by the prescription (RX) requirements. The laboratory must adhere to the precise instructions detailed on the order form. In case of any queries, the Contractor is expected to promptly contact the Provider/Specialist who placed the order.

Specific Tasks/Deliverable
Task 1: Dental Providers/Specialists are required to submit a detailed order form online, specifically tailored for each patient. Upon submission, the Contractor will engage directly with the submitting Dental Personnel (Provider, Specialist, Resident). A maximum of 50 orders per month will be issued or requested, encompassing both the Michael E. DeBakey VA Medical Center Hospital Dental Clinic and the Conroe Dental Clinic.
Task 2: The Contractor must provide the deliverables to the Dental Personnel who made the request, adhering to the timeframe indicated on the order form. In cases of potential delays, the Contractor is required to contact the submitting Dental Personnel to verify if the adjusted timeframe aligns with the patient s needs.

Task 3: The Contractor must ensure timely delivery of the deliverables to prevent potential cancellations of surgeries awaiting the surgical guides.
