Brief Description of Supplies/ Services required and the intended use/Estimated Amount: An ophthalmologic perimeter is used to diagnose and treat: glaucoma; retinal separation; optic nerves pathologies; hypertension; retinal hemorrhage; macular degeneration; malignant tumors.
Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The Haag Streit Octopus 600 is a unique instrument without competitors or generics. It can be used outside of a dark room and fits almost anywhere. It s ergonomic design allows for patients who are difficult to position for Humphry Visual Fields to sit more comfortably and the ergonomic response button is easy for patients with arthritis to use.
Description of market research conducted and results or statement why it was not conducted:
The required item is available from the OEM via the NAC. The OEM has only two SDVOSB authorized distributors.
