Description

(i) This solicitation for commercial items, prepared in accordance with the format in subpart 12.6 of the FAR and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued.

(ii) Solicitation number 285-24-RFQ-0017 applies, and is issued as a Request for Quote (RFQ).



(iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-101.



(iv) This requirement is a total Small Business set-aside and the associated NAICS is 621391 with Small Business size standard $9.0 M



(v)Items/Services

1. Monthly Diabetic foot clinic onsite, one day a month.

2. One pair DM shoes

3. Maximum, 3 DM inserts



(vi)Description of requirements

The Catawba Service Unit will contract with a state licensed Orthotist/Pedorthist to provide a monthly foot clinic to our Diabetic population at the Catawba Service Unit. The contractor will evaluate and design a treatment plan for each individual who attends the clinic.



The plan may include; extra depth or custom shoes and /or molded inserts. One pair of shoes and up to three inserts will be provided annually, unless otherwise indicated.



Measurements will be taken at the time of the evaluation and prescribed device(s) ordered. The device(s) will be available for pick up the following month.



The contractor, along with the Service Unit will plan and provide group and /or individual education as it relates to Diabetic foot care.



The deliverables are custom prescribed orthotic footwear. One pair of shoes and up to three pair of inserts will be provided to each patient annually.



To qualify for footwear coverage, Medicare beneficiaries must have diabetes plus one of the following conditions:



Neuropathy with evidence of callus, previous or current ulcer, previous or current pre-ulcerative callus, previous, amputation, foot deformities, or poor circulation. The necessary information will be submitted on the form completed by the service unit M.D.



The proposed number of patients to be serviced is approximately 60.

The contractor shall be licensed in the state of South Carolina.

The contractor will be required to enter into an Business Associate Agreement with IHS, and pass an IHS background check and credentialing.



(vii) Performance

One Day a month onsite at the Catawba Service Unit.



The period of performance shall be awarded for the base year plus four option years.



(viii) The provision at 52.212-1, Instructions to Offerors - Commercial Items (Sep 2023) applies to this acquisition.



(ix) 52.212-2, Evaluation -- Commercial Items (Nov 2021) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1) Technical Capability; 2) Management Approach & Key Personnel; 3) Relevant Past Performance; and 4) Price. Technical and other non price factors, when combined, are equal to price.



(x) Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items (Feb 2024), with its offer. The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically in the System for Award Management (SAM) accessed through https://www.sam.gov.



(xi) FAR 52.212-4 Contract Terms and Conditions - Commercial Items (Nov 2023) applies to this acquisition. The following addendum applies: only electronical invoicing will be accepted.
