The VA is issuing this notice as a SOURCES SOUGHT or Request For Information (RFI) to determine if there are enough interested and qualified sources to obtain offers for the items listed below as Brand Name or EQUAL. This is NOT a request for quotes.

INTERESTED SOURCES SHALL RESPOND BY:

2. Whether your firm distributes the Brand Name items, or Equal items.
3. SAM UEI number.
4. NAICS (North American Industry Classification System) code that your firm uses for this product.
5. State if your firm is a large business, small business, service-disabled-veteran-owned
small business, veteran-owned small business, HUB zone 8(a), women-owned small business or a small dis-advantaged business.
6. State if your quote is using GSA pricing: If so, please provide:
