Performance Work Statement MILK AND DAIRY PRODUCTS
DELIVERIES OR PERFORMANCE:
Deliveries shall be made not less that tow (2) days per week, preferably on Monday and Thursday. If a holiday falls on one of these days, delivery on the holiday shall be made unless prior arrangements have been made with Nutrition and Food Service. All deliveries must be made after 6:00 a.m. and completion of all deliveries made by 3:30 p.m. Dairy products delivered must follow the current USDA Food Code to ensure food safety guidelines are followed for all dairy products. Quantity of dairy products needed will vary depending on the number of patients. Dairy products shall be rotated following the first-in first -out (FIFO) model. Current milk is stored in the milk Cooler and shall be stocked from left to right. Expired/damaged product shall be removed from premises by vendor on the day of dairy delivery. Empty milk crated shall be removed from premises by the vendor the day of the dairy delivery.

CONTRACT ADMINISTRATION DATA: The Contracting Officer as the agent of the Government, with authority to negotiate terms, execute legally binding agreements affecting price, quantity, or quality of performance under this contract and administer contracts on behalf of the Government. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment will be made in the contract price to cover any increase in costs incurred as a result thereof.
CONTRACTING OFFICER S TECHNICAL REPRESENTATIVE: A Contracting Officer s Technical Representative (COTR) is not required.
AGE OF PRODUCT: The finished products shall be delivered a minimum of seven (7) calendar days prior to the date coded on the individual containers. Nutritive breakdown on packaging should reflect specifications.
RECALLED AND DEFECTIVE PRODUCTS: The Contractor will immediately notify the Contracting Officer of any recalls of product or other important product safety issues. As appropriate, the Contractor will replace and / or reimburse recalled / defective products at no cost to the Government. The Contractor may be liable for costs of processing recalls, i.e., administrative, and clinical services to replace recalled / defective products.
