PROJECT NO: 660-21-704 PROJECT NAME: EHRM NextGen Wi-Fi Upgrades Salt Lake City, Utah 1. GENERAL INTENT: The Contractor shall furnish all labor, tools, materials and equipment to support installation of replacement and new Wi-Fi Access Points at the George E Wahlen Veteran Affairs Medical Center (VAMC) in Salt Lake City, Utah. The AT&T NextGen Wi-Fi Contractor is responsible for the bulk of the work under a separate OI&T contract. Please note that the scope of this work is limited and specifically as listed below. 2. STATEMENT OF WORK: Provide and install conduit, raceways, horizontal and vertical penetrations, and fire stopping to support NextGen Wi-Fi installation on the Portland VA campus as shown and described in the project drawings and specifications. A list of required conduits and/or penetrations are provided in SOW Attachment 1. Work shall include: 1. Installation of 59 Access Points. Access Points shall be provided by the VA, but the mounting brackets shall be provided by the Contractor. 2. Installation of approximately 750 linear feet of conduit, including junction boxes, pull cords, and fire stopping, throughout the Medical Center. Existing conduit paths between floors/IT rooms will not have the capacity for more cable and new conduit paths will need to be created. 3. Installation of approximately 6,000 linear feet of CAT 6A cable from APs to nearest IT rooms and terminating in a patch panel. 4. Installation of four (4) patch panels. All work will be performed during business hours, Monday thru Friday, 8 AM to 4:30 PM. 5. The Prime Contractor shall designate a minimum of one Site Safety and Health Officer (SSHO) to administer the contractor s safety program and government-accepted Accident Prevention Plan. Each subcontractor shall designate a minimum of one Competent Person (CP) in compliance with 29 CFR 1926.20 (b)(2) that will be identified as a CP to administer their individual safety programs. Also, The SSHO or an equally-qualified designated representative/alternative will maintain a presence on the site during construction operations in accordance with FAR Clause 52.236-6: Superintendence by the Contractor. CPs will maintain presence during their construction activities in accordance with above mentioned clause. A listing of the designated SSHO and all known CPs shall be submitted prior to the start of work as part of the APP with the training documentation and/or AHA. In order to comply with above, please do NOT bid for same SSHO that would or would potentially cover multiple projects on the campus. 3. PERIOD OF PERFORMANCE: 3.1 Work timelines and completion progress shall be communicated to the COR for tracking purposes. 3.2 Work described in Section 2 shall be completed within 90 calendar days of issuance of construction notice to proceed (NTP). 4. SUBMITTALS: a. The contractor shall submit the following submittals for review: 1) Product data and sample of conduit or raceway. 2) As-built drawings for all data locations and labels. The Contractor shall provide the central drawing set to the Owner at the end of the project. The marked-up drawing set will accurately depict the as-built status of the system. 7. General 7.1 This entire project shall conform to 100% of all VA standards and specifications, OSHA, AIA Construction Guidelines, Joint Commission Environment of Care Standards, and applicable building codes. The Contractor shall be responsible for the health and safety of their own employees. The VA will inform the Contractor of any hazards such as designated permit confined spaces, electrical hazards, etc. which may affect Contractor employees. The Contractor must take precautions appropriate for the safety of their employees as well as provide equipment necessary for these actions. Contractor employees will notify the VA Representative before entering a designated permit required confined space, so precautions can be taken in advance of the work. The Contractor shall adhere to VHA Directive 1192.01: Seasonal Influenza Vaccination Program for VHA Healthcare Personnel. The VHA Directive requires all HCP covered by the policy to receive the annual seasonal influenza vaccination. All contractors and subcontractors will follow COVID screenings and mask regulations per CDC policy and VHA Directives. 7.2 The Contractor shall only use Government IT equipment for purposes specifically tied to or authorized by the contractor and in accordance with VHA and SLC VAMC policy. The Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected from public disclosure by an exemption to the Freedom of Information Act. SLC VAMC owns the rights to all data and records produced as part of the contract. All deliverables under the contract are property of the U.S. Government for which SLC VAMC shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. **End of SOW**
