OFFERORS READ THIS INFORMATION FIRST FOR IMPORTANT SOLICITATION INFORMATION
This a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This is a solicitation conducted using FAR Part 13, Simplified Acquisition Procedures. This announcement constitutes the only solicitation, and a Request for Quote (RFQ) is being requested.
Solicitation Number: 36C26324Q0494.
Solicitation Description: On-Site Urology Physician (NOTE: Review the Performance Work Statement (PWS) for specific qualifications, credentials, work hours, etc)
The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2024-02 dated 01/22/2024.
This procurement is set-aside for Service-Disabled Veteran Owned Small Business (SDVOSB) entities. At the time of submission of offer and prior to award, the offeror must be verified as a small business in the System for Award Management (SAM) reflecting NAICS code 561320 and be listed in the SBA VetCert website as a certified SDVOSB firm.
The VA anticipates the award of one Firm Fixed Price (FFP), Indefinite Quantity Contract. The period of performance is from June 1, 2024 to November 30, 2024 and includes one (1) six-month option period. The option period shall be exercised at the discretion of the Government.
Solicitation Questions. All solicitation questions shall be submitted in writing via email to nicole.burski@va.gov by 4/2/2024, 11:00 AM CST.
Submission of Offer: Refer to FAR Provision 52.212-1 within the attached solicitation document for instructions on submitting an offer.
Evaluation Factors. Comparative Evaluation Process. Offerors shall submit required documentation outlined in FAR Contract Clause 52.212-2, Evaluation Factors. Offerors are responsible to review the solicitation s FAR provision 52.212-2 to determine the required documentation to be submitted with their offer.
Offer Due Date: Quotes are due to the Contracting Office by April 10, 2024 at 11:00 AM Central Time. Quotes shall be emailed to nicole.burski@va.gov prior to the solicitation closing date and time. It is the responsibility of the offeror to ensure their quote is received in full by the Contracting Officer prior to the solicitation closing date and time. Due to email file size limitations, please do not wait until the last minute to email offers.
