Fulton County, Georgia (“County”) is seeking a qualified contractor with substantial knowledge and experience to provide Mobile Wellness Clinic services. This is a Medicare benefit service for seniors age 65 and older. Through the issuance of this Request for Proposal (“RFP” and/or “Proposals”), the County is soliciting Proposals from qualified Proposers for Mobile Wellness Clinic services. Proposals provided in response to this RFP that comply with the submittal requirements set forth in Section 4.0, including all forms and certifications, will be evaluated in accordance with the criteria and procedures described in Section 5.0. Based on the results of the evaluation, the County will award the Mobile Wellness Clinic Services project to the most advantageous Proposer(s) based on the cost and the evaluation factors set forth in the RFP.

A Pre-Proposal Conference, will be held on Monday, April 10, 2023
