Important: In accordance with LA R.S. 37:2163A, Contractors license number in the appropriate
classification(s) such as Municipal and Public Works and / or 7-27 Installation of Equipment,
Machinery and Engines and/ or 7-83 Irrigation and Wastewater Systems and Pumps and / or 7-112
Well Servicing, Workovers, Completion and Plug and Abandon Well must appear on the bid opening
envelope on all projects in the amount of fifty-thousand dollars ($50,000) or more (and $1.00 or more if
hazardous materials are involved). Issues with the above classification(s) must be brought to the
attention of the Office of State Procurement prior to bid opening.
In accordance with LA R.S. 37.2163, anyone objecting to the classification must send a certified letter
to both the Louisiana State Licensing Board for Contractors and the Office of State Procurement. The
letter must be received no later than ten (10) working days prior to the day on which the bids are to be
opened.
For any bid submitted in the amount of fifty-thousand dollars ($50,000) or more, the contractor shall
certify that he is licensed and show his license number on the bid envelope.
For any bid submitted in the amount of fifty-thousand dollars ($50,000) or more, a bid security must be
attached (insurance company, bank money order, certified check, or cashiers check) in the sum of
five percent (5%) of the amount bid (including base bid and additive alternates, if any) and shall
become the property of the agency in the event the contract and bond are not executed within the time
set forth in the bid solicitation. If bid bond is used, it shall be written by a surety or insurance company
currently on the U.S. Department of the Treasury Financial Management Service List of Approved
Bonding Companies, which is published annually in the Federal Register, or by a Louisiana domiciled
insurance company with at least an A- rating in the latest printing of the A.M. Bests Key Rating Guide
to write individual bonds up to 10 percent of policy holderssurplus as shown in the A.M. Bests Key
Rating Guide or by an insurance company that is either domiciled in Louisiana or owned by Louisiana
residents and is licensed to write surety bonds. In addition, the bond shall be written by a surety or
insurance company that is currently licensed to do business in the State of Louisiana.

Specifications:
Successful vendor will provide pump testing at each of the
twenty (20) existing relief wells (RW-1 to RW-10 around
the pump station/drainage structure & RW-11 - RW-20
around Camp-C on the west side of the ring levee).
* This pump testing is needed per Corp of Engineers to
determine the well capacity and compare the results with
the original data estimated from pump tests conducted
after the installation of the relief wells RW-1 to RW-10 from
4/26/2000 to 5/5/2000 and to also determine a baseline
capacity for RW-11 thru RW-2, whose original capacity is
not currently known.
* Each well will be developed using the airlift method
utilizing a large volume air compressor for approx. 30
minutes. Prior to doing the airlift development, high
pressure water will be sued to flush the backfill drains on
the relief wells at the Pump station (RW-1 - RW-10. There
are no backfill drains on the relief wells at Camp-C RW-11
to RW-20.
* After completion of each relief well a pump test will be
conducted following the relief well pump test procedure.
* the well capacity value for each well will be estimated by
dividing the average flow value of the pump test to the
maximum amount of drawdown measured during the test.
* the obtained results of the specific capacity will be
compared to the original values. If the difference between
original values and current pump test results is 20 percent
or more, the relief well will require rehabilitation
** Vendor will provide the agency with a report of the
findings so agency and MVN Corp of Engineers can
determine if rehabilitation of any relief wells are needed in
a new seperate phase of this project.

ANY PRODUCT OR SERVICE BID SHALL CONFORM TO ALL APPLICABLE FEDERAL AND STATE LAWS AND REGULATIONS

18. PRICES .
UNLESS OTHERWISE SPECIFIED BY THE STATE OF LOUISIANA IN THE SOLICITATION, BID PRICES MUST BE COMPLETE,
INCLUDING TRANSPORTATION PREPAID BY BIDDER TO DESTINATION AND FIRM FOR ACCEPTANCE FOR A MINIMUM OF
30 DAYS. IF ACCEPTED, PRICES MUST BE FIRM FOR THE CONTRACTUAL PERIOD. BIDS OTHER THAN F.O.B. DESTINATION
MAY BE REJECTED. PRICES SHOULD BE QUOTED IN THE UNIT (EACH, BOX, CASE, ETC.) AS SPECIFIED IN
THE SOLICITATION.
19. DELIVERIES.
BIDS MAY BE REJECTED IF THE DELIVERY TIME INDICATED IS LONGER THAN THAT SPECIFIED IN THE SOLICITATION.
20. TAXES.
VENDOR IS RESPONSIBLE FOR INCLUDING ALL APPLICABLE TAXES IN THE BID PRICE. STATE AGENCIES ARE EXEMPT
FROM ALL STATE AND LOCAL SALES AND USE TAXES.
21. NEW PRODUCTS.
UNLESS SPECIFICALLY CALLED FOR IN THE SOLICITATION, ALL PRODUCTS FOR PURCHASE MUST BE NEW, NEVER
PREVIOUSLY USED, AND THE CURRENT MODEL AND/OR PACKAGING. NO REMANUFACTURED, DEMONSTRATOR, USED
OR IRREGULAR PRODUCT WILL BE CONSIDERED FOR PURCHASE UNLESS OTHERWISE SPECIFIED IN THE SOLICITATION.
THE MANUFACTURER'S STANDARD WARRANTY WILL APPLY UNLESS OTHERWISE SPECIFED IN THE SOLICITATION.
22. CONTRACT RENEWALS.
UPON AGREEMENT OF THE STATE OF LOUISIANA AGENCY AND THE CONTRACTOR, A TERM CONTRACT MAY BE EXTENDED
FOR 2 ADDITIONAL 12-MONTH PERIODS AT THE SAME PRICES, TERMS AND CONDITIONS. IN SUCH CASES, THE TOTAL
CONTRACT TERM CANNOT EXCEED 36 MONTHS.
23. CONTRACT CANCELLATION.
THE STATE OF LOUISIANA HAS THE RIGHT TO TERMINATE THE CONTRACT IMMEDIATELY FOR ANY OF THE FOLLOWING REASONS:
