Project Overview:
Repair of 2 deteriorated tiles. They are approximately parallel to the concrete double box culvert at approximately 130’ each, on each side.
Special Requirements and Bidding Information: Tama County Township, T-83N-R13W, Section 36 & T-83N-R12W, Section 31.
Project Location: Mile Marker 61.2 and Station 245+35+/-.
Until the contract is awarded the sole point of contact for this project is Jean Gustafson, jean.gustafson@iowadot.us.

All questions related to this project may be submitted through the IonWave Bid system through the cut off period for question submissions.
DOT estimated start date is to begin as soon as ground permits and completed by May 31, 2024.

1 Acceptance of Standard Terms and Conditions

Participating Agencies that interferes with fair competition or constitutes a conflict of interest.

3 Performance and Payment Bond
If the contracted amount is $25,000 or more, the successful Bidder must furnish a performance and payment bond
covering the faithful performance of 100% of the Contract and the payment of all obligations arising thereunder.
Iowa DOT will provide the Performance and Payment Bond Form 131070 upon contract award.
A Resident Commission Agent or attorney-in-fact must file a copy of the power of attorney.

4 Power of Attorney
Attorney-in-fact who signs the Proposal Guaranty and/or Performance Bond must file with each bond a certified and
effectively dated copy of the Power of Attorney.
5 Labor Relations
All Bidders, before entering into a contract with the Department, must be registered with the Division of Labor in the
Iowa Department of Workforce Development (515-281-3606) according to chapter 91C, Code of Iowa 2015. This
pertains to contractor’s who engage in the business of construction.

6 Contractor Guarantee
The Contractor must guarantee all work executed under this contract, both as the workmanship and materials, for a
period of twelve (12) months after the substantial completion date. Neither the final payment nor any provision of
the contract documents must relieve the Contractor of responsibility for faulty materials or workmanship. The
Contractor must remedy any defect thereto and pay for any damage to other work resulting therefrom, which must
appear within a period of one (1) year from the date of the final acceptance. With one month remaining in the
guarantee period, the Contractor must notify the Iowa DOT and set up a complete walk-through inspection.
All materials, items of equipment, and workmanship furnished under this division of the specifications must carry the standard
warranty against all defects in material and workmanship. Any fault due to defective or improper material, equipment, or
workmanship which may develop, must be made good, forthwith.
The Guarantee must include, but not be limited to the following elements and services:
1. Repair or replace defective materials, equipment, workmanship and installation that develops within the
guarantee period, promptly and to Iowa DOT's satisfaction and correct damage caused in making
necessary repairs and replacements, including all other damage done to areas, materials, and other
systems resulting from the failure or defect, under guarantee by and at the expense of the Contractor.
2. Replace material or equipment that requires excessive service during guarantee period, as defined
and as directed by the Iowa DOT.
3. Make all service calls, replacements, repairs and adjustments during the guarantee period without cost
to the Iowa DOT.

7 DOT Responsibility-Inspection and Supervision
All work shall be according to the approved design and shall be under the direct supervision of the Iowa DOT.
Periodic site inspections will be carried on by the Iowa DOT with the Contractor to ensure coordination of the
project.
The Iowa DOT will provide a list of items requiring inspection prior to or during installation. The Contractor is
to give the Iowa DOT notice no less than 48 hours in advance of installation.
The Iowa DOT point of contact after the contract award shall be determined.
8 Insurance Required
The resulting Contract will require the successful Contractor to maintain insurance coverage(s) of the type and in
the amounts set forth in the Standard Terms and Conditions or as otherwise specified.

1 Project Description per attached Scope of Work.
NOTE: Applicable traffic control standards shall apply and be furnished by the awarded bidder performing the
work. Awarded bidder shall determine appropriate pipe elevations to maintain flow of the tile line. Awarded bidder
shall be responsible for final grading and seeding of disturbed areas. Pipe bedding/aggregate needs shall be
furnished by awarded bidder along with pipe, fittings, and intakes. Awarded bidder performing the work shall
assume the responsibility to comply with work as outlined by the permit and it's attachments.
All applicable Iowa DOT Standards and Sections shall apply.
Installation of inspection accesses shall be completed per Standard Road Plan DR-302 (attached)
Materials shall meet the following requirements:
Steel pipe shall be new and meet the requirements of ASTM A139, Grade B; STM A 252, or ASTM A 53,
Grade B.
Steel pipe shall have a coating to inhibit rust formation
Steel pipe shall have a wall thickness of .281” or thicker Standard Road Plan TC-202 shall be followed

Disturbed areas shall be reseeded in accordance with Table 2601.03-1 found in Standard .
