Potential projects include offsite projects and complex healthcare projects located on the main campus in Rolla, Missouri. The value, scope, and duration for each project are yet to be fully determined, but include the following: *Immediate Care ($1,400,000)* Highway 72 Building ($180,000)* Telemetry Boom Removal ($317,000)* Expansion Joint replacement ($125,000)* 2020 Capital Projects (less than $5,000,000)Phelps Health intends to select the Construction Management at Risk firm that provides the best overall experience and depth of resources for pre-construction and construction services. The Construction Management team must be able to work and communicate with Phelps Health and the entire Project Team. All interested firms are to submit qualifications by 2 p.m., November 29, 2019. The submittals will be publicly opened with a member of the Phelps Health board present. Based on the selection criteria listed below (A: General Requirements (RFQ)), Phelps Health will invite no more than five CMAR candidates to submit a further response to its Request for Proposal (RFP), using the criteria set forth in B: Selection Criteria, further below.A. GENERAL REQUIREMENTS1. Firm Name, Address, Phone Number, Point of Contact2. Healthcare Experience List all healthcare projects completed in the last 5 years Provide three Owner References 3. Provide an overview of your firm's CM Capabilities Preconstruction (including Budget, Schedule, BIM) Construction (include Safety & Quality Programs) Please include your EMR (Experience Modifier Rate) for the last three years. A description of your company's ICRA (Infection Control RiskAssessment) policies. A description of your company's last three year safety rating. A description of Bonding Capabilities4. Leadership Team Organizational Chart Resumes5. Provide a summary describing why your firm is the most qualified resource and would provide the most value in assisting Phelps Health for this effort (two-page max).B. SELECTION CRITERIA Phelps Health will use the following criteria as principal considerations to evaluate RFQ submittals: Firm's Overall Qualifications and Healthcare Experience & Capabilities Leadership Team - Depth of Resources Project Controls - Safety, Quality, Budget, and Schedule Firm's Ability to Provide Value to Phelps Health Demonstration of Ability to Perform Projects Comparable in Design, Scope, and Complexity Demonstration of Good Faith Efforts to Achieve Compliance with Federal, State, and Local Affirmative Action Requirements References of Owners for Whom Construction Management has been Performed Financial Strength Qualifications of In-House Personnel Who will Manage the Project The Demonstration of Successful Management Systems Employed for the Purposes of Estimating, Scheduling, and Cost Controls.After Phelps Health selects its short list of Construction Manager at Risk applicants as described above, each such CMAR applicant must submit information in its response to Phelps Health's Request for Proposal (RFP) as to its proposed fees and price for fulfilling the general conditions. No more than five CMAR candidates will be short listed and requested to submit a RFP. Phelps Health reserves the right to waive any informalities or minor defects in the received proposals and may accept any RFQ that, in the judgment of Phelps Health, will serve the best interests of Phelps Health.
