contract that may result from this bid/proposal/solicitation.
5 Successful Bid/Proposal Response Submittal
All bids/proposals must be SUCCESSFULLY submitted through this bid system, before the due date and time,
regardless of your organization's ability to submit bids online.
It is the supplier's sole responsibility to ensure that bid/proposal documents successfully are submitted before the
due date and time.
The District's online bidding application utilizes the Internet and the World Wide Web, which is comprised of systems
that are completely out of the District's control.
It is highly recommended that the suppliers allow themselves enough time to complete a successful
submission.

1 Certificate of Non-Discrimination
3 By submission of this bid or proposal, the vendor certifies that: "The Fort Worth Independent School District does
not discriminate on the basis of sex, disability, race, religion, color, age, gender, sexual orientation, and/or national
origin in the educational programs or activities which it operates, and it is required by Title IX, Section 504, Title VII,
and the Americans With Disabilities Act not to discriminate in such a manner. This policy not to discriminate extends
to employment in and admission to such programs and activities."
Submittal to District of reasonable evidence of discrimination will be grounds for Termination of the Agreement. This
policy does not require the employment of unqualified persons.
By the signing of this Certificate, the Contractor signifies that it does not maintain or provide for its employees any
segregated facilities at any of its establishments, and that it does not permit its employees to perform their services
at any location, under its control, where segregated facilities are maintained. It certifies further that it will not

proposed Subcontractors (except where the proposed Subcontractors have submitted identical certifications for
specific time periods): Notice to Prospective Subcontractors of requirement for certification of non-
segregated facilities. ꞏA certification of non-segregated facilities, as required by the May 19, 1967 Order (32 FR.
7439, May 19, 1967) on elimination of segregated facilities, by the Secretary of Labor, must be submitted prior to
the award of a Contract exceeding $10,000.00 which is not exempt from the provisions of the Equal Opportunity
Clause. The certification may be submitted either for each subcontract or for all subcontracts during a period (i.e.,
quarterly, semiannually, or annually).
