1.0 BACKGROUND INFORMATION
Santa Clara County Superior Court is requesting a quotation from businesses interested in providing
periodic low voltage services for new pull and troubleshooting calls at Court’s facilities within Santa
Clara County.

2.0 DESCRIPTION OF SERVICES AND DELIVERABLES
Santa Clara Superior Court periodically has a need for entities to provide low voltage end to end
cabling for new installation and troubleshooting/repairing existing infrastructure during normal business
hours, off Court hours and 24/7 emergency repair within a 4-hour window. Low voltage wiring (copper
or fiber) that could support the below equipment and more.
PCs, laptops, VOIP phones, multi-functional machines (copy/scan/fax), servers, security system
equipment, telecom equipment, switches, routers, firewalls, patch panels and wireless access
points
Low voltage support for Court’s data centers and POE devices
Services may also include:
Running wire through existing conduits or floor/ceiling tracks
Installation of wire mold, raceways, trays, wiring conduits
Proper termination of wiring at wall plates, surface mount boxes, SFPs and patch panels
Thorough end to end testing and report of individual runs.
Providing wire management to organize wiring and minimize safety hazards.
Proper labeling source and destination of individual runs with labeling tape
Providing reasonable slack on request at each run to support future modifications.

On this form, the Proposer must indicate acceptance of the Terms and
Conditions or identify exceptions to the Terms and Conditions.
Note: A material exception to a Minimum Term will render a
proposal non-responsive.
The Proposer must complete the General Certifications Form and
submit the completed form with its proposal.
The Proposer must complete the Darfur Contracting Act Certification
and submit the completed certification with its proposal.
This form contains information the COURT requires in order to
process payments and must be submitted with the proposal.
For vendors claiming the DVBE incentive, the Proposer must complete
the DVBE Declaration and Bidder Declaration forms to receive the 3%
incentive. See section 13.0.
The Proposer must complete the Iran Contracting Act Certification and
submit the completed certification with its proposal.
The Proposer must complete the Unruh Civil Rights Act and California
Fair Employment and Housing Act Certification.
5.0 PAYMENT INFORMATION
Standard payment is after the Court’s acceptance of work at the completion of each project free of
punch list items.
For complex projects that may require extended length of time, progress payment is negotiable.
The court and contractor must determine and agree in writing the progressive payments on a
project-by-project basis prior to commencement.
Payments are subject to 10% payment withholding per invoice for inadequate work performance,
incomplete deliverables, and incomplete punch list.
Payment is net 30 days after acceptance of invoice.

7.0 SUBMISSIONS OF PROPOSALS
7.1 Proposals should provide straightforward, concise information that satisfies the requirements of
the “Proposal Contents” section below. Expensive bindings, color displays, and the like are not
necessary or desired. Emphasis should be placed on conformity to the RFP’s instructions and
requirements, and completeness and clarity of content.
7.2 The Proposer must include in the proposal.

8.0 PROPOSAL CONTENTS
8.1 Technical Proposal: The following information must be included in the technical proposal.
A proposal lacking any of the following information may be deemed non-responsive.
a. The Proposer’s name, address, telephone and fax numbers, and federal tax
identification number. Note that if the Proposer is a sole proprietor using his or her
social security number, the social security number will be required before finalizing a
contract.
b. Name, title, address, telephone number, and email address of the individual who
will act as the Proposer’s designated representative for purposes of this RFP.
c. For each key staff member: a resume describing the individual’s background and
experience, as well as the individual’s ability and experience in conducting the proposed
activities.
d. Acceptance of the Terms and Conditions.
(1) On Attachment 3, the Proposer must check the appropriate box and sign the form.
If the Proposer marks the second box, it must provide the required additional
materials. An “exception” includes any addition, deletion, or other modification.

(2) If exceptions are identified, the Proposer must also submit (i) a red-lined version
of the Terms and Conditions that implements all proposed changes, and (ii) a
written explanation or rationale for each exception and/or proposed change.
(3) Note: A material exception to a Minimum Term will render a proposal non-
responsive.
g. Certifications, Attachments, and other requirements.
(1) The Proposer must complete the General Certifications Form (Attachment 4) and
submit the completed form with its proposal.
(2) The Proposer must complete the Darfur Contracting Act Certification (Attachment
5) and submit the completed certification with its proposal.
(3) If Contractor is a California corporation, limited liability company (“LLC”), limited
partnership (“LP”), or limited liability partnership (“LLP”), proof that Contractor is in
good standing in California. If Contractor is a foreign corporation, (LLC, LP, or
LLP, and Contractor) conducts or will conduct (if awarded the contract) intrastate
business in California, proof that Contractor is qualified to do business and in
good standing in California. If Contractor is a foreign corporation, LLC, LP, or LLP,
and Contractor does not (and will not if awarded the contract) conduct intrastate
business in California, proof that Contractor is in good standing in its home
jurisdiction.
(4) Copies of the Proposer’s (and any subcontractors’) current business licenses,
professional certifications, or other credentials.
(5) [for solicitations of $1,000,000 or more] The Proposer must complete the Iran
Contracting Act Certification (Attachment 7) and submit the completed certification
with its proposal.
(6) (for solicitations of $100,000 or more) The Proposer must complete the Unruh Civil
Rights Act and California Fair Employment and Housing Act Certification (Attachment
8) and submit the completed certification with its bid.
8.2 Cost Proposal. The following information must be included in the cost proposal.
a. A detailed line-item budget showing total cost of the proposed services.
b. A full explanation of all budget line items in a narrative entitled “Budget Justification.”
c. A “not to exceed” total for all work and expenses payable under the contract, if awarded.
NOTE: It is unlawful for any person engaged in business within this state to sell or use any
article or product as a “loss leader” as defined in Section 17030 of the Business and
Professions Code.
9.0 OFFER PERIOD
A Proposer's proposal is an irrevocable offer for ninety (90) days following the proposal due date. In
the event a final contract has not been awarded within this period, the COURT reserves the right to
negotiate extensions to this period.
